SARASOTA -- Mrs. Phyllis Dale Smith of Sarasota, formerly of Polk County, died Friday (April 23, 2004). She was 72.Visitation will be from 10 to 11 a.m. Wednesday at Sarasota Memorial Funeral Home. Services will follow at 11 a.m. at the funeral home chapel. Burial will be in Bear Creek Hill Grove Cemetery, Dayton, Ohio.
