Phyllis M. Miller, 56, Lafayette, died at 10:14 p.m., Monday, Nov. 30, 2009, at her residence. She was born at El Paso, Texas on Sept. 7, 1953, to the late Rev. Leslie Chadwick and Helen (Hall) Chadwick, Arcadia. She married Jack Miller, Jr. He survives. Visitation will be from 10 a.m. to noon, Saturday, at Soller-Baker Lafayette Chapel, 400 Twyckenham Blvd., Lafayette. Services will follow at noon, Saturday, at the funeral home.
