FANTHOM , L. Hazel (nee Black) 1916 - 2009 With great sadness the family of Hazel Fanthom wishes to announce her passing on Saturday, September 5, 2009. Hazel was predeceased by her husband Alfred S. Fanthom, father Alfred H. Black, her mother Ethel Black (nee: Davidson) and her brother Gordon. Hazel was born on November 17, 1916 in Calgary. She lived most of her life in Calgary. She is survived by her daughters Lorraine (Melvin) Kokoski, Marie (Grant) Winter; grandchildren Garrett (Erica) Kokoski; Tracey (Blair) Torgerson; Kyle Kokoski (Patty Gallivan), Matthew (Teri-Lynn) Winter; Sarah Winter (Ryan Witteman); six grandchildren Ryan, Craig, Gracelynn, Chelsey, Charlee and Marcel. A Memorial Service will be held at FOSTER'S GARDEN CHAPEL, 3220 - 4 Street N.W., Calgary (across from Queen's Park Cemetery) on Friday, September 11, 2009 at 10:00 a.m. Interment will follow at Queen's Park Cemetery. If friends so desire, memorial donations may be made directly to Heart and Stroke Foundation of Alberta, No. 200, 119 - 14 Street N.W., Calgary, Alberta T2N 1Z6.
